What to Expect When You Choose Us

Step 1

We Become Your Primary Care Provider

Think of us as the captain of your healthcare team. We work closely with any specialists, caregivers, and loved ones involved in your care. We believe in a holistic approach to your well-being, creating a comprehensive care plan and helping to make sure everyone is on the same page.

Step 2

Initial Visit - Getting to Know You

Our first visit is all about you! We enjoy taking the time to listen to your story, learn about your life experiences, and get to know your loved ones and current life events. We also discuss your future wishes as you age. This comprehensive understanding helps us provide the personalized and holistic care approach we believe in.

Step 3

Regular Visits At Your Home

We plan regular checkups, typically every five to six weeks, depending on your current health needs. We also stay in touch with your caregivers to determine if additional visits are needed. Regular checkup visits are like a typical primary care appointment, but our visits take place in the comfort of your home, whether that’s in an assisted living facility or a private residence.

Step 4

Ongoing Coordination with Caregivers

Health needs can arise unexpectedly, so we stay connected with your caregiver between visits. If any issues come up, you or your caregivers or family can contact us to coordinate the best course of action. We offer telemedicine visits and after-hours phone support for urgent care needs and can often fit in same-day or next-day visits.

Step 5

Keeping Your Loved Ones Informed

With your consent, we keep your loved ones informed about your health status. We respond promptly and make ourselves available to provide updates to you or your power-of-attorney upon request.

We currently offer services to individuals living in private residences, licensed Adult Family Homes, and Assisted Living Facilities throughout the greater Tucson, Arizona metro area.
